Monitor Phase. a. The evaluator shall observe probationary evaluatees at least four (4) times, and tenured teachers two times (2), reasonably spaced, during the school year and more frequently if deemed necessary by either party. Observations shall be documented utilizing the evaluation observation form. b. A personal conference shall be held shortly after each observation and the evaluation form signed by both parties. A copy shall be given to the evaluatee. c. A written notation of all observations and conferences shall be made by the evaluator on the evaluation form. d. The evaluator shall counsel the evaluatee during the evaluation period on a continuous basis utilizing various communication techniques.
