Common use of Occurrence Form Clause in Contracts

Occurrence Form. Physician agrees to obtain and maintain in full force and effect at all times during the term of this Agreement, an occurrence form of professional liability insurance policy with minimum limits of One Million Three Hundred Thousand Dollars ($1,300,000) per incident and Three Million Nine Hundred Thousand Dollars ($3,900,000) in the aggregate, or such higher limits as the Corporation’s contracts and agreements may require from time to time for Physicians. Physician shall cause the Corporation to be named as an additional named insured on Physician’s professional liability insurance policy and shall, at the Corporation’s request, provide a Certificate of Insurance to the Corporation evidencing same.
