OPTIONS IN THIS SETTLEMENT. Do Nothing and Receive a Payment To receive a cash payment from the Settlement, you do not have to do anything. The actual amount you receive will depend on a number of factors. Additional information is set forth below. Exclude Yourself On or before [45 days from mailing] To exclude yourself, you must send a written request for exclusion to the Settlement Administrator as provided below. If you request exclusion, you will receive no money from the Settlement and you will not be bound by the Settlement. However, if you are also an Aggrieved Employee and exclude yourself from the Settlement Class, you will still be paid your allocation of the PAGA Payment and will remain subject to the release of the Released PAGA Claims regardless of whether you submit a request for exclusion. Instructions are set forth below in Section 7.
