OTHER INSTRUCTIONS. A. The supervisor shall sign the performance evaluation form and obtain the signature of the employee at the time of the evaluation meeting. B. The supervisor shall submit the completed form to his immediate supervisor for review and signature. C. The supervisor may make a copy of the performance evaluation form for his or her own files. D. The supervisor shall give the employee a copy of the completed evaluation and if applicable the professional development / improvement plan. E. The original copy of the evaluation and if applicable the professional development / improvement plan shall be sent to the Human Resource Office.
