Outcomes Report Clause Samples
The Outcomes Report clause requires one party to provide a detailed summary of results or progress related to a specific project, service, or set of deliverables. Typically, this involves compiling and submitting a document that outlines key achievements, metrics, or milestones reached during a defined period. By mandating regular or final reporting, this clause ensures transparency and accountability, allowing stakeholders to assess performance and make informed decisions based on documented outcomes.
Outcomes Report. By March 31 of each year, British Columbia and UBCM will provide to Canada an outcomes report that will outline the following program benefits:
a) beneficial impacts on communities of completed Eligible Projects, supported by specific outcomes examples in communities;
b) the impact of CCBF as a predictable source of funding;
c) progress made on improving Local Government planning and asset management, including development or update of Housing Needs Assessments; and
d) a description of how CCBF funding has alleviated housing pressures tied to infrastructure gaps and contributed to housing supply and affordability outcomes (further details on this requirement may be found in Schedule G – Housing Report). The outcomes report will present a narrative on how each program benefit is being met. A template and guidance document will be provided by Canada. In support of transparency and accountability of the CCBF, the following communications protocol will apply to all communications activities undertaken regarding any CCBF funding and will apply to the Parties and Ultimate Recipients. Communicating to Canadians on the use of CCBF funding is clearly linked with our joint accountability to Canadians. Compliance with this protocol will inform the timing and flow of any CCBF funding and is critical to meeting our joint commitment to transparency.
Outcomes Report. The Recipient shall account in writing for outcomes achieved as a result of the Funds through an Outcomes Report to be submitted to AMO. Specifically the Outcomes Report shall describe, in a manner to be provided by AMO, the degree to which investments in each Eligible Project are supporting progress towards achieving:
(a) beneficial impacts on communities of completed Eligible Projects; and
(b) enhanced impact of Funds as a predictable source of funding.
Outcomes Report. The Outcomes Report will report on the cumulative investments made, including information on the degree to which these investments have actually contributed to the objectives of cleaner air, cleaner water and reduced GHG emissions.

Outcomes Report. By March 31, 2018 and March 31, 2023, Ontario will provide to Canada and make publicly available an Outcomes Report that will report in aggregate on the degree to which investments are supporting the progress in Ontario towards achieving the following program benefits:
a) beneficial impacts on communities of completed Eligible Projects;
b) enhanced impact of GTF as a predictable source of funding; and,
c) progress made on improving planning and asset management in Unincorporated Areas. This includes demonstrating how Asset Management Plans are being used to guide infrastructure planning and investment decisions and how GTF funding is being used to address priority projects. The Outcomes Report will present performance data and a narrative on how each program benefit is being met. Performance measurement methodology in respect of each program benefit will be approved by Canada’s and Ontario’s Executive Leads. Performance methodology for asset management will be proposed to the Oversight Committee by Ontario.
Outcomes Report. An Outcomes Report form, as provided by the City, shall be submitted annually, unless otherwise directed by the City. The Report shall be submitted to the City no later than fifteen calendar days following the end of the year, for the most recent 12-month period for which the Agency has complete outcomes data.
Outcomes Report. By March 31, 2029, and March 31, 2034, Ontario will provide to Canada an Outcomes Report that will outline the following program benefits:
a) beneficial impacts on communities of completed Eligible Projects, supported by specific outcomes examples in communities;
b) the impact of CCBF as a predictable source of funding; and
c) progress made on improving planning and asset management in Unincorporated Areas. The Outcomes Report will present a narrative on how each program benefit is being met. A template and guidance document will be provided by Canada.
Outcomes Report. Within 90 days of the receipt of all necessary outcomes data for the full project from the DPD, DCP and CDHS, the evaluator shall release to the Governance Committee and the ▇▇▇▇▇▇ Foundation a full and final Outcomes Report triggering Outcome- based Success Payments. The Outcomes report shall: • Follow the evaluation plan. • Report detailed findings with respect to the intervention’s impact on the key outcomes in the intervention, including criminal justice and/or human services involvement, as documented in the accompanying Success Payment Triggers for Success Payment Period 2. • Include clear, easily understandable summaries regarding any statistically significant differences with respect to Outcomes achieved across the Treatment and Control Groups. • Report clearly, based on the evaluation findings, the Success Payments owed by the State under the Pay for Success Contract. • Address any other meaningful differences between the Treatment and Control Groups and show how the evaluation addressed these differences. • Describe any substantial factors that may have complicated or obscured the evaluation. • Highlight any critical findings from the evaluation that might suggest ways the impact of the program could be further strengthened. • Further show how the evaluation controlled for other relevant variables. DCP PROVIDER NETWORK MEMORANDUM OF UNDERSTANDING AND AGREEMENT This MEMORANDUM OF UNDERSTANDING AND AGREEMENT is made this day of July 1, 2018, between Savio House’s Savio Management Group, hereinafter referred to as “SMG” with its principal place of business at ▇▇▇ ▇▇▇▇ ▇▇▇▇▇▇, ▇▇▇▇▇▇, ▇▇ ▇▇▇▇▇ and Savio, hereinafter referred to as “Provider”. The Provider acknowledges SMG’s role is to be the purchasing agent for the Denver Collaborative Partnership, hereinafter referred to as “DCP”, to serve as a liaison between DCP and Providers, and manage payment and monitoring of providers, based on direction of DCP. These Providers and SMG in the context of this purchasing agent role are collectively referred to hereinafter as the “DCP Provider Network.”
Outcomes Report. The Recipient shall account in writing for outcomes achieved as a result of the Funds through an Outcomes Report to be submitted to AMO upon completion of an Eligible Project and to be made available publicly in manner consistent with financial reporting under the ▇▇▇▇▇▇▇▇▇ ▇▇▇, ▇▇▇▇ S.O. 2001 c.25 by March 31st of the following Municipal Fiscal Year.
a. The Recipient’s Outcomes Report shall report in writing on the cumulative investments made, in a manner to be provided by AMO, including information on the degree to which these investments have actually contributed to the objectives of cleaner air, cleaner water and reduced greenhouse gas emissions.
14. Section 9.2 is deleted and replaced as follows:
Outcomes Report. All RADF funds must be acquitted within eight (8) weeks of the stated project completion date by completing and submitting the online Outcome Report.
