Common use of Participant Satisfaction Survey Clause in Contracts

Participant Satisfaction Survey. 1. Conduct a participant satisfaction survey annually with program participants using the County-provided template to obtain the views about the services received. 2. Subrecipients have the flexibility to add questions or include a needs assessment. Additions to the survey must be approved by the County prior to dissemination. 3. Survey finding must be tabulated using the Participant Satisfaction Survey Results Workbook and submitted to the County via email to the ▇▇▇▇▇▇▇▇▇▇▇@▇▇▇▇.▇▇▇▇▇.▇▇▇ on or before May 30th along with a summary detailing a plan of action addressing relevant concerns of participants to improve program services. 4. Subrecipient must keep the completed surveys and the tabulated results on file.
