Participating Provider Directory. Contractor shall make its provider directory electronically available to (i) the Exchange for publication online in accordance with guidance from the Exchange, and (ii) in hard copy when potential Enrollees make such request. Contractor shall provide information describing Participating Providers in its QHP networks in a format prescribed by the Exchange on a quarterly basis to support the Exchange’s centralized provider directory containing every QHP’s network providers.

Contractor shall provide information describing Participating Providers in its SADP networks in a format prescribed by the Exchange on a quarterly basis to support the Exchange’s centralized provider directory containing every SADP’s network providers.
Participating Provider Directory. Each Participating Provider agrees to have their names, business addresses, and other pertinent information about their practice set forth in the Delta Dental Medicaid Network provider directory or other lists of Participating Providers, which are published in various media for the benefit of Enrollees. Participating Provider agrees to promptly inform DDWA of any changes, deletions or additions to the information provided for the provider directory.

Contractor shall make a hard copy of its Provider directory available to Qualified Individuals upon request, and in accordance with applicable Laws. Contractor’s Provider directory shall include either a Provider National Provider Identifier or unique Provider identifier for inclusion on the State’s enrollment transaction files.
Participating Provider Directory. BCBSNC shall make available to the Plan Administrator a directory of Participating Providers, which shall be updated no less frequently than semi- annually or as otherwise agreed by BCBSNC and the Plan Administrator at the sole expense of BCBSNC. BCBSNC shall also maintain a Provider directory that is available on an internet website and is updated no less frequently than weekly.
Participating Provider Directory. Freeman shall provide Payor on a semi-annual basis with an updated list containing the names and addresses of all Participating Providers, and shall provide Payor monthly with a list of additions and deletions to the most recent quarterly list. Freeman may, at its sole discretion, revise from time to time, the Participating Provider Directory which includes those providers who have authorized ▇▇▇▇▇▇▇ to contract on their behalf in accordance with this Agreement.

You’re encouraged to: • become involved in your healthcare by asking providers about all treatment plans available and their costs; • take advantage of the preventive health services offered under this plan to help you stay healthy and find problems before they become serious. Each member is required to select and provide the name of his or her network PCP who will provide and arrange for your health care. Your PCP provides your health care, orders lab tests and x-rays, prescribe medicines or therapies, and arranges hospitalization when necessary. You may choose one from the list of Access Blue New England network PCP providers on our website. Each enrolled member may select a different PCP. If a PCP is not chosen, we may assign one for each enrolled member. You may change your designated PCP by calling our Customer Service Department or visiting our website. Finding a PCP in our network is easy. To select a provider, or to check that a provider is in our network, please use the “Find a Doctor” tool on our website or call Customer Service.
