Patient Assistance Related Functions. For each of the Reviewed Policies and Procedures identified in Section II.A.1 above, the report shall include the following items:‌ 1. a description of the documentation (including policies) reviewed and any personnel interviewed;‌ 2. a detailed description of Good Days’ systems, policies, processes, and procedures relating to the items identified in Section II.A.1 above, including a‌ general description of Good Days’ control and accountability systems (e.g., documentation and approval requirements, and tracking mechanisms) and written policies regarding the Reviewed Policies and Procedures;‌‌‌ 3. a description of the manner in which the control and accountability systems and the written policies relating to the items identified in Section II.A.1 above are made known or disseminated within Good Days;‌ 4. findings and supporting rationale regarding any weaknesses in Good Days’ systems, processes, policies, and procedures relating to the Reviewed Policies and Procedures, if any; and‌ 5. recommendations to improve any of the systems, policies, processes, or procedures relating to the Reviewed Policies and Procedures, if any.‌

Amgen’s organizational structure as it relates to arrangements and interactions with Independent Charity PAPs, including:‌ i. the identification of those individuals, departments, or groups within Amgen (e.g., the PAP Governance Committee, legal, compliance) that have responsibility for, or involvement with, such arrangements and interactions;‌‌ ii. the respective scope and nature of the responsibilities of each individual, department, or group with responsibility for, or involvement with, arrangements and interactions with Independent Charity PAPs; iii. the identification of those individuals, departments, or groups within Amgen (e.g., the commercial business units) that are precluded from involvement with arrangements and interactions with Independent Charity PAPs; and‌ iv. methods that Amgen uses to separate Independent Charity PAP- related responsibilities from the commercial business units. b. Patient Assistance Related Functions a. PSI’s systems, policies, processes, and procedures relating to Patient Assistance Related Functions. This review shall include an assessment of the following: i. PSI’s systems, policies, and practices for ensuring that PSI and the officers, directors, employees and agents of PSI are independent from and do not have any conflicts of interest with Donors or potential Donors. ii. PSI’s systems, policies, and practices relating to disease funds, including: (a) the systems, policies, and practices that PSI uses to establish, delineate, and modify any disease fund. This includes the bases upon which PSI makes decisions about funds (as referenced in Section III.F.1.c of the IA), the definition and type of disease funds, the scope of coverage for disease funds (as referenced in Section‌‌ III.F.1.d of the IA), the Notification List practices (as referenced in Section III.F.1.f) and the drug coverage methodology used by PSI in connection with its funds (as outlined in Section III.F.1.g); and (b) PSI’s documentation and recordkeeping practices relating to disease funds as outlined in Section III.F.1.‌ iii.   • Outpatient Dental Anesthesia Services This plan covers anesthesia services received in connection with a dental service when provided in a hospital or freestanding ambulatory surgical center and: • the use of this is medically necessary; and • the setting in which the service is received is determined to be appropriate.   • Routing for Operator Services and Directory Assistance Traffic For a Verizon Telecommunications Service dial tone line purchased by VarTec for resale pursuant to the Resale Attachment, upon request by VarTec, Verizon will establish an arrangement that will permit VarTec to route the VarTec Customer’s calls for operator and directory assistance services to a provider of operator and directory assistance services selected by VarTec. Verizon will provide this routing arrangement in accordance with, but only to the extent required by, Applicable Law. Verizon will provide this routing arrangement pursuant to an appropriate written request submitted by VarTec and a mutually agreed-upon schedule. This routing arrangement will be implemented at VarTec's expense, with charges determined on an individual case basis. In addition to charges for initially establishing the routing arrangement, VarTec will be responsible for ongoing monthly and/or usage charges for the routing arrangement. VarTec shall arrange, at its own expense, the trunking and other facilities required to transport traffic to VarTec’s selected provider of operator and directory assistance services.

  • Information Services Traffic 5.1 For purposes of this Section 5, Voice Information Services and Voice Information Services Traffic refer to switched voice traffic, delivered to information service providers who offer recorded voice announcement information or open vocal discussion programs to the general public. Voice Information Services Traffic does not include any form of Internet Traffic. Voice Information Services Traffic also does not include 555 traffic or similar traffic with AIN service interfaces, which traffic shall be subject to separate arrangements between the Parties. Voice Information services Traffic is not subject to Reciprocal Compensation as Local Traffic under the Interconnection Attachment. 5.2 If a Reconex Customer is served by resold Verizon Telecommunications Service or a Verizon Local Switching UNE, subject to any call blocking feature used by Reconex, to the extent reasonably feasible, Verizon will route Voice Information Services Traffic originating from such Service or UNE to the Voice Information Service platform. For such Voice Information Services Traffic, unless Reconex has entered into an arrangement with Verizon to ▇▇▇▇ and collect Voice Information Services provider charges from Reconex’s Customers, Reconex shall pay to Verizon without discount the Voice Information Services provider charges. Reconex shall pay Verizon such charges in full regardless of whether or not it collects such charges from its own Customers. 5.3 Reconex shall have the option to route Voice Information Services Traffic that originates on its own network to the appropriate Voice Information Services platform(s) connected to Verizon’s network. This trunk group will be utilized to allow Reconex to route Voice Information Services Traffic originated on its network to Verizon.   • Dependent Care Assistance Program The County offers the option of enrolling in a Dependent Care Assistance Program (DCAP) designed to qualify for tax savings under Section 129 of the Internal Revenue Code, but such savings are not guaranteed. The program allows employees to set aside up to five thousand dollars ($5,000) of annual salary (before taxes) per calendar year to pay for eligible dependent care (child and elder care) expenses. Any unused balance is forfeited and cannot be recovered by the employee.

  • CUSTOMER SERVICE FUNCTIONS The Servicer shall handle all Customer inquiries and other Customer service matters according to the same procedures it uses to service Customers with respect to its own charges.
