PERFORMANCE HISTORY. 1. In the past five (5) years, has your firm been found civilly liable, either in a court of law or pursuant to the terms of a settlement agreement, for defaulting or breaching a contract with a government agency? If Yes, use Attachment A to explain specific circumstances. 2. In the past five (5) years, has a public entity terminated your firm's contract for cause prior to contract completion? If Yes, use Attachment A to explain specific circumstances and provide principal contact information. 3. In the past five (5) years, has your firm entered into any settlement agreement for any lawsuit that alleged contract default, breach of contract, or fraud with or against a public entity? If Yes, use Attachment A to explain specific circumstances. 4. Is your firm currently involved in any lawsuit with a government agency in which it is alleged that your firm has defaulted on a contract, breached a contract, or committed fraud?
