This policy applies only to accidents and losses dur- ing the policy period shown on the declarations page and which occur within any state, territory, or posses- sion of the United States of America, or any province of Canada, or while a covered vehicle is being trans- ported between their ports. This policy, your insurance application (which is made a part of this policy as if attached hereto), the declara- tions page, as amended, and endorsements to this policy issued by us contain all the agreements between you and us. Subject to the following, its terms may not be changed or waived except by an endorsement issued by us. Only the Named Insured may request any changes that require the execution of a state-mandated form in order to effectuate a change in the policy or coverages.
