POPULATION TO BE SERVED. In accordance with the Contract, Contractor is required, within the limits of the Contractor’s service capacity, to serve individuals who meet the financial and clinical eligibility criteria for Seriously Emotionally Disturbed (SED) children and adolescents eligible for services as described in the DARHMA manual.
POPULATION TO BE SERVED. CONTRACTOR shall provide services promoting safe and stable 20 families to birth, kinship, blended, adoptive, and ▇▇▇▇▇▇ families with 21 children, ages birth through eighteen (0-18) years, who are at risk and/or 22 experiencing child abuse and/or neglect, or families living in poverty or 23 economic hardships, domestic violence, unemployment, teen pregnancy, and 24 unhealthy parenting families receiving child welfare services, including 25 families in the family reunification and/or adoption process; homeless 26 families, unaccompanied homeless youth, and those at risk of homelessness; 27 non-minor dependents ages eighteen through twenty-one (18-21), who are being 28 served by child welfare or probation agencies and who are under the 1 jurisdiction of the Orange County Juvenile court; military families (active 2 and veteran); and persons with disabilities. The population to be served as 3 defined in this Paragraph shall hereinafter be referred to as "PARTICIPANTS" 4 or “FAMILIES.”
POPULATION TO BE SERVED. 11 CONTRACTOR shall provide services to children referred by SSA. Services will be 12 provided to Orange County children ages six (6) through eighteen (18) years, under court 13 jurisdiction, in out of home placements, including but not limited to children placed with resource 14 families or at Orangewood Children and Family Center (OCFC). The population to be served will 15 collectively be referred to as “YOUTH/YOUTHS.”
POPULATION TO BE SERVED. 16 CONTRACTOR shall provide services promoting safe and stable families 17 specified below to families with children, ages birth through eighteen (0-18) 18 years, who are at risk, or have a history of abuse and/or maltreatment, or 19 live in poverty, or receive child welfare services that reside in the City of 20 Orange, California, and surrounding communities within Orange County. The 21 population to be served as defined in this paragraph shall hereinafter be 22 referred to as "PARTICIPANTS."
POPULATION TO BE SERVED. 15 CONTRACTOR shall provide services to children birth (0) to eighteen (18) years with 16 abuse or neglect issues who have been referred by Social Services Agency (SSA)/Children and
POPULATION TO BE SERVED. 11 1. 1 CONTRACTOR agrees to provide residential care and treatment 12 services to ▇▇▇▇▇▇ Youth/NMD’s referred to CONTRACTOR by COUNTY pursuant to 13 the terms and conditions set forth herein, in accordance with the ▇▇▇▇▇▇ 14 Youth’s/NMD’s Admission Agreement and CONTRACTOR’s Program Statement, approved 15 by CCLD and incorporated herein by reference, as it currently exists or may 16 hereafter be amended.
POPULATION TO BE SERVED. CONTRACTOR shall provide Family Resource Center (FRC) services, as contained in Paragraph 5 of this Exhibit, to: birth, kinship, blended, adoptive, and Resource Families with children, ages birth to eighteen (0-18) years, who are at risk of or are experiencing child abuse and neglect; families who are living in poverty or suffering economic hardship, domestic violence, unemployment, teen pregnancy, and unhealthy parenting; families involved with and/or receiving child welfare services; non-minor dependents ages eighteen (18) to twenty- one (21), who are being served by child welfare or probation agencies and who are under the jurisdiction of the Orange County Juvenile Court; homeless families, unaccompanied homeless youth, and those families at-risk of homelessness; military families; and persons with disabilities. The population to be served as defined in this Paragraph shall hereinafter be referred to as “PARTICIPANTS” or “FAMILIES.” CONTRACTOR shall provide FRC services primarily to those PARTICIPANTS residing in the city of Huntington Beach and surrounding communities.
POPULATION TO BE SERVED. ECSOC Services are provided to children ages birth (0) through five (5) years, who enter the child welfare system. The population served is hereinafter referred to as CHILDREN.
POPULATION TO BE SERVED. In accordance with the Contract, Contractor is required within the limits of the Contractor’s service capacity, to serve individuals who meet the financial and clinical eligibility criteria for an adult or child for Chronic Addiction (CA) and is eligible for services as described in the DARMHA Manual.
POPULATION TO BE SERVED. A. In accordance with the Contract, Contractor is required, within the limits of the Contractor’s service capacity, to serve individuals who meet the financial and clinical eligibility criteria for an adult who is Seriously Mentally Ill (SMI) and is eligible for services as described in the DARMHA manual.
B. Contractor shall:
1) Assure the availability of, and provide the necessary full continuum of care as defined in IC 12-7-2-40.6, to adults who are SMI eligible for registering in DARMHA in this Exhibit; and
2) Provide DMHA supported care to special populations listed in this Exhibit within the limits of the Contractor’s service capacity.
