Preliminary Closeout Materials. The Grantee must submit the following Preliminary Close- Out Materials along with the Termination of Disbursements Letter: 1. Final Choice Neighborhoods Budget; 2. Actual HOPE VI Cost Certificate (Cost Certificate) (Form HUD-53001-A), or a Choice Neighborhoods successor form if created, which summarizes the information on the Financial Status Report and serves as the document that officially closes out the grant.
