Preventable Conditions. No payment will be made by Subcontractor to a Provider for provider preventable conditions, as identified in the State Program. Provider shall identify and report to Subcontractor any provider preventable conditions in accordance with 42 C.F.R. §§ 434.6(a)(12)(i) and (ii) and 42 C.F.R. § 447.26(d).
