The Agency further agrees that all reportable incidents of abuse and neglect, as defined in section 488(1)(a)-(h) of the Social Services Law, and all reportable incidents of injury or illness or death indicated in paragraph 5 on page 32 of this Agreement shall be submitted to the New York State Justice Center for the Protection of People With Special Needs.
