Procedure for Requesting Leaves of Absence. All requests for leaves of absence of less than thirty (30) days other than sick leave, funeral leave, vacation leave, and leave to attend employee organization meetings, shall be made by the employee, on forms provided by the City, to his/her immediate supervisor in accordance with the following schedule:
