Programmatic Reports Clause Samples

The Programmatic Reports clause requires one party to provide regular, structured updates or summaries regarding specific activities, progress, or outcomes related to a program or project. Typically, these reports may include data on milestones achieved, resource usage, challenges encountered, and next steps, and are often submitted on a predetermined schedule such as monthly or quarterly. This clause ensures that all stakeholders remain informed and aligned, promoting transparency and accountability throughout the duration of the program.
Programmatic Reports. 4.2.1 Quarterly Statistical (Performance) Reports. GRANTEE shall deliver to the OAG quarterly statistical reports no later than the thirtieth (30th) day of each month following the end of each quarter covered by the term of this contract. The four (4) quarters, within each year covered by the term of this contract, end respectively on the last day of the months of November, February, May and August. Accordingly, quarterly statistical reports, for each respective quarter, are due on or before December 30, March 30, June 30, and September 30.
Programmatic Reports. CONTRACTOR shall submit monthly Programmatic reports to ADMINISTRATOR. These reports shall be in a format approved by ADMINISTRATOR and shall include but not limited to, descriptions of any performance objectives, outcomes, and or interim findings as directed by ADMINISTRATOR. CONTRACTOR shall be prepared to present and discuss the programmatic reports at the monthly and quarterly meetings with ADMINISTRATOR, to include an analysis of data and findings, and whether or not CONTRACTOR is progressing satisfactorily and if not, specify what steps are being taken to achieve satisfactory progress.
Programmatic Reports. 4.2.1 Quarterly Statistical (Performance) Reports. The four (4) quarters end respectively on the last day of the months of November, February, May and August. Accordingly, quarterly statistical reports, for each respective quarter, are due on or before December 30, 2017, March 30, 2018, June 30, 2018, and September 30, 2018. Contents of Quarterly Statistical Reports. The quarterly statistical reports shall contain, at a minimum, information on the following statistical measures: a. Targets as established by the OAG; b. Outputs and Outcomes as established by the OAG; and c. Program Impact Narratives.
Programmatic Reports. Subrecipient Entity is required to submit programmatic reports in the format required by the State. Subrecipient Entity shall file reports following a reporting schedule not less than quarterly but more frequently if required by the State Agency. Subrecipient Entity will promptly provide State Agency with all information necessary for State Agency to comply with the reporting requirements contained in Treasury’s SFRF Compliance and Reporting Guidance, including without limitation the quarterly Project and Expenditure Reports and the annual Recovery Reports.
Programmatic Reports. CITY may be required to submit census reports to COUNTY as requested by COUNTY. These reports shall be on a form acceptable to, or provided by COUNTY. COUNTY may request additional program reports of CITY in order to determine the quality and nature of services provided hereunder. COUNTY will be specific as to the nature of information requested, and may allow up to thirty (30) calendar days for CITY to respond to request.
Programmatic Reports. The Provider agrees to submit programmatic reports as requested and in the format required by DHS.
Programmatic Reports. Programmatic reports are prepared and submitted to IYF on a quarterly basis using a standard format provided in the grant agreement. The programmatic report provides an opportunity for the recipient to provide a formal progress update on accomplishments for the quarter, including updated monitoring and evaluation data, progress against leverage targets, as well as significant challenges and issues that require further discussion. More frequent reports or updates may be required based on the needs of the program. Financial reports are submitted on a monthly basis using the template provided in the grant agreement. The monthly financial reports provide information on the use of funds during the prior period and may include a projection of expenses expected for the following period; financial reports must be signed by an authorized representative of the grantee organization. Fund disbursements are determined based on an analysis of funds previously disbursed, reported expenditures, and expected funding needs for the following month. A sample financial reporting template is provided in Attachment 9. Administrative reporting is required based on additional information required by IYF and USAID. Inventory reporting and reporting on Value Added Tax (VAT) is required on an annual basis, and other reporting requirements may be incorporated into the grant agreement, such as annual submissions of an updated board list. Specific instructions, reporting templates, and reporting schedules are provided in the grant agreement.
Programmatic Reports. The Quarterly Programmatic Report and Demographics report (See Attachment B, Quarterly Programmatic Report and Demographics) have been updated for FY 2021. All other reporting requirements remain in effect.
Programmatic Reports. The county must keep track of all partnerships it makes, obstacles it encounters in coordinating the program, and strengths and weaknesses of the initiative it rolled out, and provide this information to the Department.  The county must agree to track each individual’s participation in the program, engagement in services, and progress towards recovery from the inception of the program to one year, and provide this information to the Department.  Data to be collected and reported will be determined based on the program design adopted in each county. For example, counties may be required to track information about each individual encountered through the program, including: demographic information; date of encounter; whether the offer of treatment was accepted; the type of treatment accepted; insurance information; contact information; treatment provider(s); dispositions in any criminal prosecution; and treatment outcomes at 8 weeks, 3 months, 6, months, and 12 months.
