Common use of Programmatic Clause in Contracts

Programmatic. 1. CONTRACTOR shall submit programmatic reports to ADMINISTRATOR, as indicated below, on a form acceptable to or provided by ADMINISTRATOR, which will be received by ADMINISTRATOR no later than twenty (20) calendar days following the end of the month/quarter being reported unless otherwise specified. Mental Health Programmatic reports will include the following: a. A description of CONTRACTOR's progress in implementing the provisions of this Contract, b. Report of placement and movement of Clients along the continuum of services using guidelines for monthly report of the number of 5150 participants, c. Voluntary and involuntary hospitalizations and special incidences, d. Vocational programs, educational programs, including new job placements, Clients in continuing employment. e. Reporting of the numbers of Clients based upon their level of function in the MORs Level system, f. Chart compliance by percentage of compliance with all Medi-Cal records, in County of Orange, Health Care Agency Contract MA-042-18010321 addition to any pertinent facts or interim findings, staff changes, status of Licenses and/or Certifications, changes in population served and reasons for any such changes. g. CONTRACTOR statement whether the program is or is not progressing satisfactorily in achieving all the terms of this Contract, and if not, shall specify what steps will be taken to achieve satisfactory progress. 2. CONTRACTOR shall document all adverse incidents affecting the physical and/or emotional welfare of Clients, including but not limited to serious physical harm to self or others, serious destruction of property, developments, etc., and which may raise liability issues with COUNTY. CONTRACTOR shall notify COUNTY within twenty-four (24) hours of any such serious adverse incident. 3. CONTRACTOR shall advise ADMINISTRATOR of any special incidents, conditions, or issues that adversely affect the quality or accessibility of Client-related services provided by, or under contract with, the COUNTY as identified in the HCA P&Ps.
