Common use of Proof of Absence Clause in Contracts

Proof of Absence. The Superintendent shall require the teacher to furnish a signed statement to the effect that absence was caused by illness or due to any of the foregoing causes. He/she may require the name of the physician and the date he/she was consulted if medical attention was required. Falsification of such a statement shall be grounds for disciplinary action, including dismissal.
