Protocol Compliance. The Investigator will conduct this study in compliance with the protocol provided by the Sponsor and given approval/favorable opinion by the IRB/IEC and the appropriate Regulatory Authority(ies). Changes to the protocol will not be made without agreement of the Sponsor Medical Monitor. All changes to the protocol will require IRB/IEC approval prior to implementation, except when necessary to eliminate an immediate hazard to study subjects or when the change involves only logistical or administrative aspects of the study (e.g., change in Sponsor Medical Monitor or telephone number). The IRB/IEC may provide, if applicable regulations permit, expedited review and approval/favorable opinion for minor changes in ongoing studies. The Sponsor will submit all protocol changes to the appropriate Regulatory Authority in accordance with the governing regulations. In situations requiring a departure from the protocol, the Investigator or other physician in attendance will contact the Sponsor Medical Monitor by telephone, e-mail or fax. If possible, this contact will be made before implementing any departure from the protocol. In all cases, contact with the Sponsor Medical Monitor must be made as soon as possible in order to review the situation and agree on an appropriate course of action. The case report form and source document will describe any departure from the protocol and the circumstances requiring it.
Protocol Compliance. CONTRACTOR shall comply with the current Placer County 5150 Protocol and the specific policies and procedures developed to assist CONTRACTOR in fulfilling its contractual agreement.
Protocol Compliance. The Investigator is responsible for compliance with the protocol at the investigational site. The Investigator is also responsible for reporting all issues of protocol non-compliance to the respective IEC and to the Sponsor. A representative of the Sponsor will make frequent contact with the Investigator and his/her research staff and will conduct regular monitoring visits at the site to review subject and study device accountability records for compliance with the protocol, e.g., subject eligibility criteria, volume of product injected, procedures performed, and follow-up visit schedule.
Protocol Compliance. Substantive changes in the protocol include alterations that affect the safety of patients or that alter the scope of the investigation, the scientific quality of the study, the experimental design, doses, assessment variable(s), the number of patients treated, or the patient selection criteria. These changes must be implemented via formal written protocol amendment only upon joint approval by the Sponsor and Investigator. Additionally, a protocol amendment must be reviewed and approved by the IRB prior to implementation. If a protocol amendment results in changes to the ICF, the revised form must also be approved by the Sponsor and the IRB prior to use. Emergency departures from the protocol that eliminate an apparent immediate hazard to a particular patient, and that are deemed crucial for the safety and wellbeing of that patient, may be instituted for that patient only. The Investigator or other attending physician will also contact the Sponsor as soon as possible in the case of such a departure. These departures do not require pre-approval by the IRB; however, the IRB and the Sponsor must be notified in writing as soon as possible after the departure occurs. Furthermore, the Investigator will document in the patient’s record and the electronic case report form (eCRF) the reasons for the departure from the protocol and the ensuing events.
Protocol Compliance. Caterer shall comply with all applicable COVID-19 rules and protocols for the Facilities as set by the Park District, County, State, or other authority, which are updated from time to time. Caterer responsibilities include, but are not limited to: • Caterer employee training. • Customer education including current rules, enforcement, and penalties, including loss of security deposit. • Inclusion of COVID-19 rules and protocols in customer contractsCompliance with vaccine mandates for employeesMonitoring and implementation of vaccine or negative test checks at guest arrival time of each reservation. Caterer shall set a high standard for COVID-19 compliance and safety. Failure to do so, may result in suspension or termination of this agreement.
Protocol Compliance. (1) Unless the Board determines otherwise based on the specific facts and circumstances, a covered QFC is deemed to comply with this subpart if it is amended by the universal protocol or the U.S. protocol. (2) A covered QFC will be deemed to be amended by the universal protocol for purposes of paragraph (a)(1) of this section notwithstanding the covered QFC being amended by one or more Country Annexes, as the term is de- fined in the universal protocol.

When immediate deviation from the protocol is required to eliminate an immediate hazard(s) to patients, the Investigator will contact La Jolla and/or the Medical Monitor, if circumstances permit, to discuss the planned course of action. Any departures from the protocol must be fully documented on the appropriate CRF and in the source documentation.
Protocol Compliance. Upon enrollment, a Pharmacist Program Coordinator designated by the pharmacy must review and sign the Pharmacist Naloxone Dispensing Protocol and Agreement (page one). This Agreement must be renewed every two years thereafter, or upon a change in the designated Pharmacist Program Coordinator. By signing the Protocol and Agreement, the designated Pharmacist Program Coordinator attests/agrees that pharmacist and patient education are met in accordance with the protocol.

  • PCI Compliance Company shall not connect to or utilize any computer network or systems of the Aviation Authority, including, without limitation, for transmission of credit card payments. Company shall be solely responsible for providing and maintaining its own computer networks and systems and shall ensure its system ensure its system used to collect, process, store or transmit credit card or customer credit card and/or personal information is compliant with all applicable Payment Card Industry (“PCI”) Data Security Standard (“DSS”). 1. Company shall, within 5 days, notify the Aviation Authority of any security malfunction or breach, intrusion or unauthorized access to cardholder or other customer data, and shall comply with all then applicable PCI requirements. 2. Company, in addition to notifying the Aviation Authority and satisfying the PCI requirements, will immediately take the remedial actions available under the circumstances and provide the Aviation Authority with an explanation of the cause of the breach or intrusion and the proposed remediation plan. Company will notify the Aviation Authority promptly if it learns that it is no longer PCI DSS compliant and will immediately provide the Aviation Authority with a report on steps being taken to remediate the non-compliance status and provide evidence of compliance once PCI DSS compliance is achieved. 3. Company, its successor’s and assigns, will continue to comply with all provisions of this Agreement relating to accidents, incidents, damages and remedial requirements after the termination of this Agreement. 4. Company shall ensure strict compliance with PCI DSS for each credit card transaction and acknowledges responsibility for the security of cardholder data. Company will create and maintain reasonable detailed, complete and accurate documentation describing the systems, processes, network segments, security controls and dataflow used to receive, process transmit store and secure Customer’s cardholder data. Such documentation shall conform to the most current version of PCI DSS. 5. Company must maintain PCI Certification as a bankcard merchant at the Airport. Company is responsible, at Company’s own expense, to contract and pay for all quarterly, annual or other required assessments, remediation activities related to processes within Concessionaire’s control, analysis or certification processes necessary to maintain PCI certification as a bankcard merchant. 6. PCI DSS - Company shall make available on the Premises, within 24 hours upon request by the Aviation Authority, such documentation, policies, procedures, reports, logs, configuration standards and settings and all other documentation necessary for the Aviation Authority to validate Company’s compliance with PCI DSS as well as make available to the individuals responsible for implementing, maintaining and monitoring those system components and processes. Requested logs must be made available to the Aviation Authority in electronic format compatible with computers used by the Aviation Authority. 7. Evidence of PCI DSS Compliance – Company agrees to supply their PCI DSS compliance status and evidence of its most recent validation of compliance upon execution of the Contract. Company must supply to the Aviation Authority evidence of validation of compliance at least annually to be delivered along with the Annual Certification of Fees in accordance with Article 5.C. of this Agreement.

  • FERPA Compliance In connection with all FERPA Records that Contractor may create, receive or maintain on behalf of University pursuant to the Underlying Agreement, Contractor is designated as a University Official with a legitimate educational interest in and with respect to such FERPA Records, only to the extent to which Contractor (a) is required to create, receive or maintain FERPA Records to carry out the Underlying Agreement, and (b) understands and agrees to all of the following terms and conditions without reservation:
