Provide instructors. Provide qualified instructors to teach all prescribed courses related to and supportive of the Program and provide appropriate classroom instruction to the Students prior to their participation in the Program. Inform the Program Participants to abide by Affiliate policies and procedures. Inform the Students, the Program faculty members, and any University employees involved with the Program (collectively, the “Program Participants”) that they are responsible to comply with the Affiliate’s policies and procedures regarding the Program or Clinical Experiences at Affiliate Facilities, including maintaining the confidentiality of patient information and preventing the unauthorized disclosure of such information in accordance with the Health Insurance Portability and Accountability Act of 1996, Pub.L. 104-191, 110 Stat. 1936, enacted August 21, 1996 as amended by Health Information Technology for Economic and Clinical Health Act of the American Recovery and Reinvestment Act of 2009, Pub.L. 111-5, 123 Stat. 226, February 17, 2009, and their implementing regulations, such as the Security Rule and the Privacy Rule (see 45 C.F.R. 160 and 164), as said acts, laws, statutes, rules, and regulations may be amended from time to time (hereafter collectively referred to as “HIPAA”), together with other applicable state and federal laws and Affiliate policies and procedures. Patient information access restricted to Program purposes only. The Students shall not have access to or have the right to review any medical record, or health care record, except where necessary in the regular course of the Program. Improper handling/disclosure may result in dismissal from the Program. The discussion, transmission, or narration in any form by the Students of any patient information of a personal nature, medical or otherwise, is forbidden except as a necessary part of the Program and any violations of this prohibition, if properly established, shall constitute grounds for immediate dismissal of the Students from the Program.
