Provider Certification. The manager/designee and employee are responsible for the 13 proper administration of the sick leave benefit. Verification from a licensed health care provider may 14 be reasonably required to substantiate the health condition of the employee or family member for 15 leave requests as allowed under Washington State Law. 16 8.12.1. Unless specifically instructed otherwise for the same injury or illness, the 17 employee shall promptly notify the appropriate work unit Superintendent or designee, by telephone 18 or otherwise, each day off due to illness. If an employee is on a special work shift, particularly where 19 a relief replacement is necessary if he/she is absent, he/she shall notify the appropriate work unit 20 Superintendent or designee as far in advance as possible of his/her scheduled time to report for work.
