The Provider Identification Number clause establishes the requirement for a healthcare provider or entity to use a specific identification number when submitting claims or conducting transactions. Typically, this number is assigned by a regulatory authority or payer, such as a National Provider Identifier (NPI) in the United States, and must be included on all relevant documentation and billing forms. Provider Identification Number. Provider must be registered as an approved service provider with AHCCCS and must have a National Provider Identifier (NPI) (if eligible for a NPI).
Provider Identification Number. Providers serving Medicaid Members must enter into and maintain a Medicaid provider agreement with HHSC or its agent to participate in the Medicaid Program, and must have a Texas Provider Identification Number (TPIN). The true and correct unique identification number of each Designated Borrower that is a Foreign Subsidiary and a party hereto on the Closing Date that has been issued by its jurisdiction of organization and the name of such jurisdiction are set forth on Schedule 5.17.

  • Tax Identification Number All deposits to the Accounts shall be subject to the Escrow Agent's receipt of a valid tax identification number for the Company, Manager or Potential Investor, as applicable.
