Common use of Qualifying Events Clause in Contracts

Qualifying Events. Individuals who don’t enroll in this plan during a designated open enrollment period may later enroll in this plan outside of an open enrollment period only if one of the following is met: • Birth of a newborn child • Marriage or entering into a domestic partnership, including eligibility as a dependent • Placement for adoption of a child of the subscriber or enrolled spouse, also applies to children placed in ▇▇▇▇▇▇ care • Loss of employer sponsored coverage • A loss of Medicaid or other public program providing health benefits • A loss of coverage due to a dissolution of marriage or termination of domestic partnership • A loss of coverage due to a change in residence and your existing health plan does not provide coverage in your new area. • Loss of COBRA benefits. • Loss of coverage on The Exchange, due to an error by The Exchange, the issuer or HHS Please see ▇▇▇▇▇▇▇.▇▇▇ or if you enrolled through The Exchange, contact The Exchange for information on required documentation for your qualifying event. When we receive your completed enrollment application and any required subscription charges within 60 days of the qualifying event, coverage under this plan will become effective on the first of the month following receipt of your enrollment application or we are notified of enrollment by The Exchange. If we don’t receive your completed enrollment application within 60 days of the date of the qualifying event, please see Open Enrollment Period.
