Common use of Re-credentialing Clause in Contracts

Re-credentialing. Provider shall cooperate with the re-evaluation of their credentials at such intervals, as DentaQuest shall determine, but not more frequently than every three years. Such evaluation may take into account a review of Provider's past performance and practice patterns, and a review of dental records and evaluations pertaining to Provider's participation in the delivery of dental care.

Provider Participating Facility and Providers shall cooperate with the re-evaluation of their credentials at such intervals, as DentaQuest shall determine, but not more frequently than every three two years. Such evaluation may take into account a review of ProviderParticipating Facility's past performance and practice patterns, and a review of dental records and evaluations pertaining to Providerthe Participating Facility's participation in the delivery of dental medical care.
