Common use of Record of Attendance Clause in Contracts

Record of Attendance. CONTRACTOR shall maintain daily records of student service provided, including the name of student, name of CONTRACTOR’S employee who rendered the service, and the amount of time of such service. The daily record of attendance is input into the Cayen SST management system daily. CONTRACTOR shall send a monthly attendance report to parents/guardians of student. They may send a copy to the school administrator if requested. A paper copy of the daily record of attendance for each DISTRICT student using the Cayen generated form is submitted with first monthly invoice. The CONTRACTOR has the option of scanning remaining attendance sheets or sending hard copies for submission CONTRACTOR must notify parent when student’s attendance becomes inconsistent before the student has missed four consecutive sessions.
