Records of Panel Decisions on Proposals of Discipline. In cases involving a proposal of discipline, if the Panel finds that no discipline is warranted, no records of the grievance proceedings (including the Panel’s decision) shall be placed in the Faculty Member’s personnel file. If the Panel upholds the proposal of discipline, the notice of investigation, notice of suspension or administrative leave (if any), preliminary findings documentation and exhibits, response to preliminary findings (if in written format), the original proposal letter, requests for mediation and grievance panel, the Panel’s decision and the final resolution of the matter shall be placed in the Faculty Member’s personnel file. If the Panel’s decision requires a revision, the revision shall also be included in the Personnel file. No records of the grievance proceedings other than those noted
