REGISTR SMLUV. Sponsor akcnowledges that the Provider, as a state contributory organization, is a statutory organization within the meaning of the Act. 340/2015 Coll., on the Register of Contracts, as amended, and the Parties agree to the publication of this contract in the Register of Contracts. REGISTR SMLUV. The PARTIES hereby acknowledge and agree that the INSTITUTION shall be required to publish the AGREEMENT and its potential amendments in accordance with Act No. 340/2015 Coll., on the Register of Contracts at ▇▇▇▇▇://▇▇▇▇▇▇▇.▇▇▇.▇▇/ to fulfill its legal obligations and solely for such purpose, SPONSOR agrees with the public disclosure of a redacted version of the AGREEMENT, provided that the extent of the redaction shall be agreed with the SPONSOR prior to the respective public disclosure to avoid the disclosure of SPONSOR CONFIDENTIAL INFORMATION (e.g. APPENDIX I of the AGREEMENT) and personal data. The INVESTIGATOR agrees to her/his name being published on the Public Administration Portal in connection with the STUDY and in accordance with the Act on the Register of Contracts. Publication shall be ensured by the INSTITUTION, which shall publish, make available or provide for publication solely the version of this AGREEMENT or any part thereof which will be prepared and provided for this purpose by CRO/SPONSOR in a machine- readable electronic format and submitted by email to no later than at the date of execution of this AGREEMENT. CRO/SPONSOR shall delete from the AGREEMENT to be published all information regarding confidential information, personal data and business secrets as defined by the Act No. 89/2012 Coll, Civil Code, except for information publicly available at the time of the conclusion of this AGREEMENT and/or any amendment (excluded information and agreed exclusion of information). As part of the process of publishing of this AGREEMENT pursuant to Act No. 340/2015 Coll. REGISTR SMLUV. SPONSOR undertakes to deliver to the INSTITUTION a redacted version of the AGREEMENT, approved by SPONSOR for the registry of contracts pursuant to the Act no. 340/2015 Coll., on special conditions for the effectiveness of some contracts, the disclosure of these contracts and contracts register; not later than on the date of the last signature of the AGREEMENT. The INSTITUTION undertakes to publish such modified version of the AGREEMENT within 5 days from the AGREEMENT signature by the INSTITUTION. If such modified version of the AGREEMENT is not disclosed by the INSTITUTION in the agreed timeframe, SPONSOR is authorized to publish such redacted version of the AGREEMENT. ZADAVATEL se zavazuje, že dodá ZDRAVOTNICKÉMU ZAŘÍZENÍ redigovanou verzi SMLOUVY určenou ke zveřejnění v registru smluv, dle zákona č. 340/2015 Sb., o zvláštních podmínkách účinnosti některých smluv, uveřejňování těchto smluv a o registru smluv, a to nejpozději ke dni posledního podpisu SMLOUVY. ZDRAVOTNICKÉ ZAŘÍZENÍ se zavazuje, že modifikovanou verzi SMLOUVY zveřejní nejpozději do 5-ti dnů od podpisu SMLOUVY ZDRAVOTNICKÝM ZAŘÍZENÍM. Nezveřejní-li ZDRAVOTNICKÉ ZAŘÍZENÍ SMLOUVU v dohodnutém termínu, je ZADAVATEL oprávněn redigovanou verzi SMLOUVY zveřejnit.

REGISTR SMLUV. Notwithstanding the foregoing, Institution, Investigator and IQVIA hereby acknowledge that the Institution is obliged to publish this Amendment pursuant to Act No. 340/2015 Coll., on Agreements Register. Any information which constitutes trade secret of either party is exempted from such publication. All terms and conditions of the Agreement not expressly amended by this Amendment remain in full force and effect. REGISTR SMLUV. Bez ohledu na výše uvedené, Poskytovatel zdravotních služeb, Hlavní Zkoušející, Zadavatel, Roche a Smluvní výzkumná organizace tímto berou na vědomí, že tento Dodatek č. 4 bude uveřejněn v souladu ▇▇ ▇▇▇. č. 340/2015 Sb., o registru smluv. Za uveřejnění dle předchozí věty odpovídá Poskytovatel zdravotních služeb. Takovémuto uveřejnění nepodléhají ty údaje, které tvoří obchodní tajemství některé ze smluvních stran. Za uveřejnění Dodatku č. 4 odpovídá Poskytovatel zdravotních služeb. Poskytovatel zdravotních služeb vyrozumí Smluvní výzkumnou organizaci o uveřejnění Dodatku č. 4v registru smluv tak, že ve formuláři používaném k uveřejnění smlouvy ▇▇▇▇ adresu jako emailovou adresu, na kterou má být zaslána notifikace o uveřejnění. Není-li Dodatek č. 4 uveřejněn Poskytovatelem zdravotních služeb ve lhůtě 5 pracovních dní od jeho podpisu všemi smluvními stranami, jsou k jeho uveřejnění oprávněni Smluvní výzkumná organizace, Roche či Zadavatel. Předpokládaná hodnota finančního plnění dle Smlouvy ve znění Dodatku č. 4 činí přibližně 2 099 000,- Kč. This amendment No 4 is drawn up in the Czech and English language versions. In case of any dispute the Czech language version shall prevail. Tento Dodatek č. 4 je vyhotoven v českém jazykovém znění a v anglickém jazykovém znění, přičemž přednost má české jazykové znění. All the confidential information, all personal data and business secrets, which, by agreement between PRA bere na vědomí, že Poskytovatel jakožto státní příspěvková organizace je povinným subjektem ve smyslu zákona č. 340/2015 Sb., o registru smluv, ve znění pozdějších smluv a obě smluvní strany souhlasí s uveřejněním této smlouvy v registru smluv. Poskytovatelem bude zveřejněna redigovaná verze, která bude schválena oběma smluvními stranami a poskytnutu PRA. REGISTR SMLUV. Zadavatel souhlasí s uveřejněním této Smlouvy a jejích náležitostí Zdravotnickým zařízením v rozsahu požadovaném za účelem splnění povinností uložených jí platnou a účinnou právní úpravou, a to zejména zákonem č. 340/2015 Sb., o registru smluv, ve znění pozdějších předpisů a zákonem č. 106/1999 Sb., o svobodném přístupu k informacím, ve znění pozdějších předpisů. Smluvní strany se dohodly, že Zdravotnické zařízení uveřejní ▇▇▇▇▇ této Smlouvy, kterou mu za tímto účelem připraví a poskytne Zadavatel nebo PRA, nejpozději ke dni podpisu této Smlouvy. Ve Smlouvě pro uveřejnění v registru smluv nebudou uveřejněny jakékoli části Smlouvy, které obsahují zejména obchodní tajemství Zadavatele a osobní údaje. Kromě těchto budou, veškeré finanční a rozpočtové informace redigovány před jakoukoli Fakultní nemocnice Bulovka / prof. MUDr. ▇▇▇▇▇▇▇▇▇ ▇▇▇▇▇, CSc.

REGISTR SMLUV. Sponsor/PRA hereby acknowledges that Institution shall be obliged to publish this Agreement in accordance with Act No. 340/2015 Coll., on the Contract Register. Zadavatel/PRA tímto berou na vědomí, že zdravotnické zařízení je povinno zveřejnit tuto smlouvu v souladu se zák. č. 340/2015 Sb., o Registru smluv. REGISTR SMLUV. In accordance with the law 340/2015 Coll. on Registry of Contracts, this Agreement and/or any amendment shall be published on the Ministerial Contract Registry within thirty (30) days from last signature. The Parties agree that PROVIDER shall publish this Agreement, its Exhibits and any future amendments, and shall limit its disclosure to the information required by law. 16.1 V souladu se zákonem č. 340/2015 Sb., o registru smluv, bude tato smlouva a/nebo její dodatky zveřejněna v registru smluv ve lhůtě třiceti (30) dnů od připojení posledního podpisu. ▇▇▇▇▇▇ souhlasí s tím, aby POSKYTOVATEL zveřejnil tuto smlouvu, její přílohy a budoucí dodatky a omezil její zveřejnění jen na informace požadované ze zákona.
