Common use of Report Formats and Schedule Clause in Contracts

Report Formats and Schedule. 9.4.1 Records shall be maintained in forms and by methods that facilitate flexible use of data contained in them to structure reports, as needed. Reports are intended to compile recorded data into useful forms of information that can be used to, among other things: 9.4.1.1 Determine and set rates, and evaluate the financial efficacy of operations; and 9.4.1.2 Evaluate past and expected progress towards achieving goals and objectives; and 9.4.1.3 Determine needs for adjustment to programs; and 9.4.1.4 Evaluate customer service and complaints. 9.4.2 The County may at no cost to itself request that Franchisee provide such additional information in the reports set forth below as County deems necessary or appropriate to meet its needs, including provision of AB 939 report information. 9.4.3 Franchisee may propose report formats that are responsive to the objectives and audiences for each report. The format of each report shall be subject to approval by County. 9.4.4 Monthly reports shall be submitted within ten (10) calendar days after the end of the report month. Quarterly reports shall be submitted within fifteen (15) calendar days after the end of the quarter. Quarters end on March 31, June 30, September 30 and December 31. 9.4.5 At the request of County, but not more then once every six (6) months, Franchisee shall report the number of set outs by service type and geographic area to the Director.
