Common use of Reporting and Notification Clause in Contracts

Reporting and Notification. The Contractor shall: 1. Ensure that the System for Award Management (▇▇▇) record is active and current; 2. Ensure the Dun and ▇▇▇▇▇▇▇▇▇▇ (D.U.N.S.) number is accurate; 3. Notify the DCFS in writing no later than 30 calendar days from the date of the execution of the EI program contract with the D.U.N.S. number and confirmation that the ▇▇▇ record is active and current; 4. Notify the DCFS within 30 calendar days of any future change in the agency name or address; and, 5. By July 1, 2016, provide to the DCFS the staff information required in Exhibit E, and inform the DCFS throughout the year if these contacts change.
