Members must fill out the following applicable forms in connection with leave under this Article: 10.4.1 “Request for Family or Medical Leave Form” prepared by the District to be eligible for leave. 10.4.2 Medical certification - either for the member’s own serious health condition or for the serious health condition of a child, parent, spouse, or designated domestic partner.
