Common use of Responsibilities of Covered Entity Clause in Contracts

Responsibilities of Covered Entity. Covered Entity shall notify Business Associate of any (i) changes in, or withdrawal of, the authorization of an individual regarding the Use or Disclosure of PHI provided to Covered Entity pursuant to 45 C.F.R. § 164.508, to the extent that such changes may affect Business Associate’s Use or Disclosure of PHI; or (ii) restrictions on Use and/or Disclosure of PHI as provided for in 45 C.F.R. § 164.522 agreed to by Covered Entity, to the extent that such restriction may affect Business Associate’s Use or Disclosure of PHI.

Responsibilities of Covered Entity. With regard to the use and/or disclosure of PHI by Business Associate, Covered Entity shall notify hereby agrees to do the following: 2.2.1 Inform Business Associate of any (i) changes in, or withdrawal of, limitations in the authorization form of an individual regarding the Use or Disclosure notice of PHI provided to privacy practices that Covered Entity provides to individuals pursuant to 45 C.F.R. § 164.508164.520, by posting on Covered Entity’s website, to the extent that such changes limitation may affect Business Associate’s Use use or Disclosure disclosure of PHI; . 2.2.2 Inform Business Associate of any changes in, or (ii) restrictions on Use and/or Disclosure of PHI as provided for in 45 C.F.R. § 164.522 agreed revocation of, the permission by an individual to by Covered Entityuse or disclose PHI, to the extent that such restriction limitation may affect Business Associate’s Use use or Disclosure disclosure of PHI.
