Prior to initiation of service, the Contractor Project Manager shall submit to the agency, an IPM Plan for each building or site which will attend be approved by the agency contact. The Contractor shall be on site to initiate service within an agreed upon number of informational and status meetings and, when appropriate, call and lead such meetingsworking days following notice of approval. Such meetings may include If the Project Management TeamIPM Plan is incomplete or disapproved, the Contract Administrator, Contractor shall have an agreed upon number of working days to submit revisions.
1. Problems and potential problems for each location;

Pest management and preventative management shall include, and be applied to all locations, all non-wood destroying insects including but not limited to roaches (all species), crickets, silverfish, pavement ants, centipedes, millipedes, ground beetles, spiders, wasps, yellow jackets, hornets, bees, fleas, cicada killers, box elder bugs, earwigs, clothes moths, flies, ants, grain insects, pill bugs, sow bugs, and members of the gnat family. Chemicals to be used in this service shall be registered with the New Hampshire Division of Pesticide Control. The Contractor shall minimize the use of pesticides whenever possible. These applications shall be restricted to unique situations where no alternative measures are available or practical and non- chemical options have been exhausted. The Contractor shall not use any pesticide until after inspections or monitoring indicate the presence of pests that exceed action thresholds and non-chemical control methods or action have not reduce the pest population to below the action threshold. The Contractor shall provide a written report explaining the identity of the target pest, the need for such treatment, the time and specific place of treatment, the pesticide to be used, the method of application, what precautions should be taken to ensure employees safety, and the steps taken to ensure the containment of the pesticide to the site of application. The Contractor shall employ the least hazardous material, most precise application technique, and minimum quantity of pesticide necessary to achieve control. All equipment and chemicals must be in strict compliance with the New Hampshire Pesticides Controls statute, the Rules of New Hampshire Pesticide Control Boards, the Occupational Safety and and other regulatory agencies. The Contractor used at the agency. The Contractor shall provide current labels for all pesticides to be used as well as brand names of pesticide application equipment, rodent bait boxes, pest monitoring devices, pest surveillance and detection equipment, and any other pest control devices or equipment that may be used to provide services. The Contractor may be requested to perform emergency service(s) that are beyond routine service requests. The Contractor shall respond to these exceptional circumstances and complete the necessary work within two (2) business days after receipt of the request. The Contractor shall furnish the Department of Corrections, New Hampshire State Prison the following services: NHSP requires the Contractor to complete a Contractor Orientation as part the IPM plan. The orientation is required for all technicians that provide pest control services to NHSP. The contractor shall a minimum of two (2) technicians who have completed the orientation available. This orientation shall be provided by NHSP and the contractor shall attend at no cost to the state. The Contractor shall provide training to NHSP staff. This training shall teach staff on best practice methods in identifying and controlling pests. This training shall be provided as part of the preventative pest control process and shall be provided at no cost to the state. The Contactor shall provide scheduled and non-scheduled pest control to rid and keep clean all the present buildings; buildings may be added or deleted. NHH may require the Contractor to rid of pests not listed in the fixed price section. This service shall be provided at an hourly rate. The Contractor shall be compensated for any traps that may be required, at a rate of cost plus twenty-five (25) percent over the Contractor Invoices shall contain all appropriate information detailing the list and net prices and amount discounted. Emergency Services shall be provided within twenty-four (24) hours of contact. Services shall be provided within two (2) business days of contact. The Contractor shall provide all pest control supplies and equipment as dictated herein. Every effort shall be made by the Contractor to significantly reduce the use of pesticides whenever the Contractor Pest Control Services Manual. Traps should be placed in areas most likely to be frequented by pests but should be strategically located so as not to interfere with operations. All trap placements should be recorded on a detailed Trap Management Log. The Trap Management Log shall be filed within the Pest Control Services Manual. When vacuuming pests or pest control material/residue, a HEPA vacuum shall be used. The Contractor shall dispose of unused or waste pesticides in accordance with applicable State and Federal laws and regulations. The Contractor shall be responsible for inspecting all buildings and disposal areas for the purpose of locating problem areas. Problem areas may include, but are not limited to, wet locations, food source areas, structure issues, and penetration spots. All findings must be reported in writing via the Commercial Service and Inspection Report to the facility contact or designee listing problem areas and corrective measures to be taken. The Contractor shall take a proactive approach to reducing the risk of pest infestations at NHSP. The Contractor shall consult with NYULH regarding the availability and cost of the Latest Models, provide as NYULH intends to incorporate the services set forth hereinLatest Models into the Contract Documents whenever feasible in NYULH’s sole discretion. These services The Contractor shall diligently keep track of equipment availability and obtain commitments from manufacturers and suppliers to ensure delivery within thirty (30) days of Contractor placing its order. To ensure the use of Latest Models as directed by NYULH, equipment will not be ordered more than sixty (60) days prior to the actual installation date. The equipment shall be provided delivered directly to the Project site or to a warehouse acceptable to NYULH for staging or storage prior to delivery to the Project site. If delivered to a warehouse, the Contractor will be responsible for the coordination of the delivery to the warehouse and the subsequent coordination and delivery of equipment from the warehouse to the Project site. The Contractor shall be responsible for the cost of all warehousing, risk of loss, and delivery of the equipment from the warehouse to the Project site. Fabricated and assembled equipment shall be wrapped and sealed in polyethylene and appropriately boxed for shipment. Standard components shall be shipped in manufacturer's original packing. Boxes shall clearly indicate the equipment contained, "front", "top", "fragile", nature of components and Project location. The Contractor shall identify and ▇▇▇▇ all equipment, materials and component parts as being dedicated to this Contract and the property of NYULH and shall cause all manufacturers of the equipment to do the same. All equipment and component parts shall not constitute inventory of the Contractor or any manufacturer of the equipment, and no creditor of the Contractor or manufacturer shall have any rights therein. The Contractor shall maintain, and cause all manufacturers of the equipment to maintain all equipment and component parts free of liens, claims or encumbrances of any nature and shall not use such equipment or component parts as security for any financing or other transaction. Any payment to the Contractor for manufactured and suitably stored equipment not yet delivered to the Project site as may be agreed to by NYULH in its sole discretion shall be conditioned upon submission by the Contractor of bills of sale or such other procedures satisfactory to NYULH to establish NYULH’s title to such materials or equipment or otherwise protect NYULH’s interest. The Contractor warrants that it has the ability to assign and shall assign all manufacturers’ warranties on the equipment to NYULH. The Contractor warrants that it will promptly complete and file all necessary forms with the applicable manufacturers to ensure recognition of all warranties NYULH may be entitled to. NYULH shall be entitled to invoke all rights and remedies contained in any manufacturer documents or materials and will be considered a third-party beneficiary with respect to Contractor’s agreements with any of its equipment manufacturers or other contractors. The draft fieldwork methodology shall address how the Contracted Services Provider(s) will conduct field investigations, which shall document historic-age properties. The Contracted Services Provider(s) shall develop a draft public involvement plan. The public involvement plan shall identify potential interested parties in the county and outline efforts to involve them in the research and review of the specific requirements of this SOW Agreement, provide the services set forth hereinhistoric resources survey. These services parties may include, but are not limited to, the county historical commission (CHC), certified local government (CLG) staff, local and statewide historic preservation organizations, and local history museums, libraries, or repositories. The public involvement plan shall incorporate at least two (2) stakeholder meetings held within the county to be surveyed. The public involvement plan shall identify the location of the first stakeholder meeting and propose one (1) primary and one (1) alternate date and time; the date of the second stakeholder meeting will be dependent on the completion of the survey fieldwork. The Contracted Service Provider(s) shall ensure the availability of the proposed location for the primary and alternate times. The first stakeholder meeting shall be provided led by the Contracted Service Provider(s) who will provide information on the purpose and goals of the historic resource survey, the research design, the preliminary fieldwork methodology, the draft historic context, and the draft geographic survey scope. The first stakeholder meeting shall provide an opportunity for the stakeholders to express their views on the historic resources survey. The Contracted Service Provider(s) shall be prepared to collect information that the stakeholders might bring to the meeting, by means of scanning, copying, or photographing. The second stakeholder meeting shall be led by the Contracted Service Provider(s) who will provide information on the results of the historic resources survey report, including the historic context, fieldwork, and evaluations.
