Common use of Self-Disclosure Clause in Contracts

Self-Disclosure. The Contractor shall require all Persons to self-report within three (3) Business Days of adjudication to the Contractor any adjudication of guilt as described in i. above for the Disqualifying Offenses. The Contractor shall immediately disallow that Person Access to any State of Florida Data or from directly performing Services under the Contract. Additionally, the Contractor shall require that the Person complete an annual certification that he or she has not received an adjudication of guilt as described in i. above for the Disqualifying Offenses and shall maintain that certification in the employment file.
