Common use of Service Capacity Clause in Contracts

Service Capacity. 5.7.1. Using the form provided by Great Rivers, the Contractor shall submit to Great Rivers a roster of all clinical staff. The roster shall be updated monthly and updates to the roster are due by the 10th of each month. The roster should identify each staff member’s name, credentials, role/job title, working locations, contact information, language, if accepting new enrollees and if the staff member is a specialist as defined by WAC. If there are providers who can be accessed on a case by case basis, information on how to access these providers must be included. 5.7.2. At a minimum the Contractor shall: 5.7.2.1. Maintain the ability to provide an intake evaluation at an Enrollee’s residence, including adult family homes, assisted living facilities or skilled nursing facilities, including to persons discharged from a state hospital or evaluation and treatment facilities to such placements when the Enrollee requires an offsite service due to medical needs. 5.7.2.2. Provide or purchase age, linguistic and culturally competent community behavioral health services for Enrollees for whom services are medically necessary and clinically appropriate consistent with the Medicaid state plan and the Federal 1915(b) Behavioral Health Waiver. 5.7.2.3. Annually complete a demographic review of individuals in services and develop a cultural services plan. Cultural Services plan should include age, gender, homelessness, and ethnicity. 5.7.2.3.1. The Cultural Service Plan is due annually on August 1.
