Service User Records. (1) The Provider must maintain Service Users' Records and other related information, including in relation to Pharmaceuticals Dispensed to Service Users, in accordance with its legal and professional obligations. (2) The Provider must transfer information about Service Users to a clinical information system repository maintained by or on behalf of Health NZ, as required by Health NZ, provided that Health NZ provides access to the repository at no cost to the Provider, and the transfer of information complies with the Provider’s and Health NZ’s obligations under the Health Information Privacy Code 2020.
Service User Records. (1) The Provider must maintain Service Users' Records and other related information, including in relation to Pharmaceuticals Dispensed to Service Users, in accordance with its legal and professional obligations. (2) The Provider must transfer information about Service Users to a clinical information system repository maintained by or on behalf of Te Whatu Ora, as required by Te Whatu Ora, provided that Te Whatu Ora provides access to the repository at no cost to the Provider, and the transfer of information complies with the Provider’s and Te Whatu Ora’s obligations under the Health Information Privacy Code 2020.
Service User Records. Record keeping (1) The Provider must maintain Service Users' Records and other related information, including in relation to Pharmaceuticals Dispensed to Service Users, in accordance with its legal and professional obligations.
Service User Records. 31.1 The Provider shall create, maintain, store and retain Service User Records for all Service Users, including without limitation Service User Records that are held by the Provider, Service User Records that are shared by the Provider and relevant providers of social care services, and Service User Records that are held by Service Users. The Provider shall retain such records for periods of time in accordance with the Law on retention of such records. 31.2 Subject to compliance with the Law, the Provider shall at the reasonable request of the Commissioner promptly transfer or deliver a copy of the Service User Record held by the Provider for any Service User for which the Commissioner is responsible to a third party provider of healthcare or social care services designated by the Commissioner. 31.3 The Provider shall promptly notify the Commissioner of the identity and contact details of its Caldicott Guardian and Senior Information Risk Owner as soon as practicable after such appointments are made or varied.
Service User Records. 4.1 If the Provider is not also providing Professional Advisory Services to a Service User in accordance with this Agreement, the Provider must: (a) maintain a transaction log of the Pharmaceuticals Supplied in accordance with this Schedule to the Service User, by NHI Number; and (b) make the log transaction available to the Authorising Provider, to enable that authorising provider to populate the Service User's medication profile.
Service User Records. B15.1. The Provider shall create, maintain, store and retain Service User health and social care records for all Service Users. The Provider must retain Service User records for the periods of time required by ▇▇▇ and securely destroy them thereafter in accordance with any applicable Guidance.‌ B15.2 The Provider shall:-‌ (a) use Service User records solely for the execution of the Provider’s obligations under this Contract; and‌ (b) give each Service User full and accurate information regarding his/her treatment and Services received.‌

The Provider must retain Service User Records for a period of at least ten (10) years from the expiry of the Agreement. and securely destroy them thereafter in accordance with any applicable guidance. The Provider must: use Service User records solely for the execution of the Provider’s obligations under this Agreement; and give each Service User full and accurate information regarding his/her support and services received.

The Provider must retain Service User records for the periods of time required by ▇▇▇ and securely destroy them thereafter in accordance with any applicable Guidance. B13.2. The Provider must: a) use Service User records solely for the execution of the Provider’s obligations under this Contract; and b) give each Service User full and accurate information regarding his/her treatment and Services received. B13.3. The Provider must at all times during the term of this Contract have a Caldicott Guardian and shall notify the Authority of their identity and contact details prior to the Service Commencement Date. If the Provider replaces its Caldicott Guardian at any time during the term of this Contract, it shall promptly notify the Authority of the identity and contact details of such replacements. B13.4. Subject to Guidance and where appropriate, the Service User records should include the Service User’s verified NHS number.

  • User Data We will maintain certain data that you transmit to the Services for the purpose of managing the performance of the Services, as well as data relating to your use of the Services. Although we perform regular routine backups of data, you are solely responsible for all data that you transmit or that relates to any activity you have undertaken using the Services. You agree that we shall have no liability to you for any loss or corruption of any such data, and you hereby waive any right of action against us arising from any such loss or corruption of such data.
