Common use of SERVICES COORDINATION Clause in Contracts

SERVICES COORDINATION. The CONTRACTOR will provide a housing specialist to work with a participant to develop an individualized housing and services plan, appropriate to the participant’s needs. This plan may include, but is not limited to, focusing on sobriety, alcohol and drug-free housing, life skills training, public benefits and entitlements, medical care, and legal concerns. The CONTRACTOR will require that the participants meet with their housing specialist at least once quarterly to discuss the progress in their housing plans and to determine what adjustments are needed in order to achieve independent living and self-sufficiency.

SERVICES COORDINATION. The CONTRACTOR will provide to the Program a participant housing specialist specialist/case manager to work with a participant participants to develop an individualized housing and services service plan, appropriate to the participant’s needs. This plan may include, but is not limited to, to focusing on on: sobriety, alcohol and drug-free housing, life skills trainingreceiving supportive services, public benefits and entitlements, medical careaccessing mainstream benefits, and addressing legal concerns. The CONTRACTOR will require that the participants meet with their housing specialist specialist/case manager at least once quarterly annually to discuss the progress in their housing plans and service plan to determine what adjustments are needed in order to achieve maintain independent living and self-sufficiency.
