Sick Leave Pool. There is hereby established a pool of sick leave days for use by teachers. The sick leave pool shall be used in the event of major illness or disability. That would include intermittent absences due to follow-up care related to the initial condition. Except as related to the underlying serious medical condition, sick leave pool days are not intended for medical or dental appointments. 7. The application form will include a written statement from the employee's physician certifying that the employee is incapable of performing his/her duties as a result of the prolonged illness or disability. Employees would be eligible for a number of days depending on longevity of sick leave pool membership. Sick Leave Pool. There is hereby established a pool of sick leave days for use by teachersParticipation 1. If the Bargaining Unit employee was hired after school started, the employee has 20 days after the hire date to enroll in the pool. The member making a request for use of the sick leave pool shall submit such information as the Cooperative Review Board shall request. The decision of the Cooperative Review Board will be rendered in writing, a copy sent to the member and a copy placed in the member’s file. 12. The Cooperative Review Board will determine the eligibility of the member for use of the sick leave pool with reference to the following criteria: a. The member must have exhausted his/her sick leave. b. The provisions of this section shall apply only to illness/disability of the employee, spouse, child, grandchild, sibling, or parent of the employee or his/her spouse. A Faculty member may apply for a maximum of 20 days credit at any one time. A maximum of 40 days may be granted in any 12-month period to an individual Faculty member. 4. Participating Faculty members may resign from the pool at any time. Upon resignation from the pool or from the College, all contributions to the pool and rights to receive leave credits from the pool shall be forfeited. 5.
