Submission of Claim Form. The Settlement Class Member shall sign and submit a completed Claim Form in the form of Exhibit 2 hereto together with all other documentation, if any, set forth below by regular First Class mail, fax or by electronic mail in Portable Document Format (“PDF”) to the Claims Administrator.
