SUBMISSION SCHEDULE AND REPORTING REQUIREMENTS. A. Grantee will submit all documents identified below to the System Agency by the applicable due date outlined below. The following reports below must be submitted to System Agency through Clinical Management For Behavioral Health Services (CMBHS) and/or any alternative method required by System Agency. Grantee is required to maintain access to required systems or platforms for the term of this Grant Agreement. B. If the due date is on a weekend or a holiday, the due date is the following business day. C. All reporting communications should include ▇▇▇▇▇▇▇’s Contract Number, legal entity name, and purpose in the email subject line. D. Grantee will submit CMBHS Security Attestation Form biannually, according to dates set by the System Agency. E. Grantee shall respond to System Agency requests and provide information and/or data meeting federal and state legislative requirements as requested and within the timelines specified. F. Grantee will comply when System Agency may require additional deliverables that do not constitute an additional cost in excess of the existing funding, to comply with federal and/or state mandates. System Agency will provide written notice to the Grantee as soon as practicable but no later than ten (10) business days prior to the effective date of additional deliverables. G. Submission requirements will survive the termination or expiration of the contract. H. Ensures that access to CMBHS is restricted to authorized users. All users are required to update their passwords every 90 days. The following reports below must be submitted to CMBHS components and/or functionality, in accordance with System Agency through Clinical Management For Behavioral Health Services (CMBHS) instructions. Grantee will use the updated components and/or any alternative method required functionality as directed by System Agency. Grantee is required to maintain access to required systems or platforms for the term of this Grant Agreement. B. If the due date is on a weekend or a holiday, the due date is the following business day. C. All reporting communications should include ▇▇▇▇▇▇▇’s Contract Number, legal entity name, and purpose in duty to submit documents survives the email subject linetermination or expiration of this Contract. D. C. Grantee will meet required number of clients served per year based on funded amount; D. Perform and submit CMBHS Security Attestation Form biannuallyGovernmental Performance and Results Act (GPRA) assessments for all participants who are engaged in treatment services. Assessments must be conducted face to face, according to dates set unless a waiver is granted by the System Agency. E. Grantee 1. The primary counselor shall respond to System Agency requests and provide information and/or data meeting federal and state legislative requirements as requested and conduct the GPRA assessment immediately after the client is stabilized on medication or within the timelines specifiedfour weeks after admission, whichever is sooner. F. 2. The GPRA assessments will be completed at intake, six-month follow-up, and discharge. Additionally, the six-month follow-up must be conducted between months five through seven, depending on the client’s availability. Grantee will comply when System Agency may require additional deliverables that do not constitute an additional cost in excess of the existing fundinguse CMBHS to conduct, document and enter assessments as close to comply with federal and/or state mandatesreal time as possible.
