THE DEPARTMENT AGREES TO. 1. PROVIDE VENDOR TRAINING AND INSTRUCTION. The Department shall provide training and instruction to each Vendor: a. Prior to or at the time of the first Agreement between the Vendor and Department; b. Annually to at least one representative of the Vendor, as specified by the Department. Training shall be: (1) On a date, time and location designated at the Department’s sole discretion; and (2) On at least one alternative date also designated at the Department’s discretion; c. During scheduled and no-notice Vendor compliance reviews that Program representatives shall conduct from time to time throughout the term of the Agreement; d. In periodic Vendor newsletter updates reporting program changes, outlining Program requirements, or emphasizing key Program components; e. Upon a Vendor’s request, at a mutually convenient time, to provide individual or facility training on designated portions of the Program that may include but are not limited to Program rules, food instrument transaction and payment procedures, and inventory requirements.
THE DEPARTMENT AGREES TO. 1. Make available such personnel and facilities, including equipment, buildings, and land under its control, as may be mutually agreed upon for execution of the program. 2. Cooperate in research, education, in-service training, public relations, and demonstration on approved projects. 3. Cooperate through the Unit program in carrying forward approved research projects on fishery and wildlife resources. For furtherance of Unit operations, the Department agrees to provide annually, through University accounts, a minimum of $40,000 to be used for basic operational expenses of the Unit (i.e. equipment and supply purchases, travel of Unit personnel and students, student stipends, etc.) and to augment research projects as mutually agreed to by the Cooperators. This amount may be supplemented by additional funds or in-kind contributions of services or utilities for the conduct of research projects requested by the Department and mutually agreed upon by the Cooperators. The Department will periodically reexamine the amount of its annual basic contribution and may make such adjustment as deemed appropriate after consideration of current economic conditions and revenues available.
THE DEPARTMENT AGREES TO. Reimburse the School Food Authority for the full cost of the beverages served free to children eligible under terms of this contract during the fiscal year for which the contract is in effect. Such payment shall be a “lump sumamount payable in the following fiscal year. If the state appropriation is insufficient to pay the full amount of aid to all schools, then payments shall be prorated among the schools entitled to the aid.

Follow-up visit with Dilated Fundus Examination as needed. This includes the review of the clinical course, assessment, adjustment of treatment recommendations, and consultation with ▇▇▇▇▇▇▇ medical and nursing staff as needed. Routine Follow-up on a bi-weekly basis. Provided to patients with established optometric diagnosis to document the course of treatment, monitor medications/prescriptions and testing/laboratory results. 2. All of the services described above must be provided on the grounds of CTPHC. The optometrist will comply with all hospital policies and procedures in the delivering of services. 3. CTPHC will provide clerical and nursing support to the optometrist for services provided at the hospital. 4. The optometrist will respond to consults/referrals and provide services bi-weekly (except during the optometrist scheduled vacation time) of the clinic's request for services or upon emergent service. 5. The optometrist will provide necessary proof of licensure, certification, valid BLS/CPR certification and malpractice insurance coverage to CTPHC and will be responsible for following through with all QI/RC and JCAHO requirements related to the delivery of his service at CTPHC. 10. The optometrist will sign in and out at the CTPHC medical clinic. All consults and bills/invoices will be signed and monitored by the clinic's nurse manager and the clinic's medical director.   • Department Head A. Within ten (10) business days from his/her receipt of the decision resulting from the previous level, the employee may appeal to the Department Head using the original copy of the grievance. B. Within ten (10) business days from the receipt of the employee's grievance, the Department Head or his/her designated representative who has not been involved in the grievance in prior levels shall make a thorough review of the grievance, meet with the parties involved and give a written decision and the reasons therefore to the employee and the Union representative. However, the Department Head or designate is not limited to denying a grievance for the reasons stated at any previous level in the procedure. Upon request, a copy of the decision will be given to the Union representative. C. If the Department Head or his/her designated representative fails to give a decision within the specified time limit, the Union shall have the option of referring a grievance alleging a violation of the negotiated agreement between the parties to arbitration. D. On matters that are not subject to arbitration pursuant to Section 8 hereafter, the written decision of the Department Head or his/her designated representative shall be final.

  • Department Heads Department heads shall normally be tenured and hold the rank of Associate Professor or Professor in one of the departments to be served, unless mutually agreed to by the departmental faculty and administration. 3.1.2.1 When it becomes known that a department head position will become vacant, or if the department head position has already become vacant, or if a new department is created, the ▇▇▇▇ of the college shall meet with the department faculty within two (2) weeks to discuss qualifications and expectations for the position. Employees shall be compensated for a minimum of one (1) hour of such work regardless of the length of the climbing assignment.
