Common use of Transfer Order Clause in Contracts

Transfer Order. The written document issued and signed by the CDCR/CCHCS patient and/or DJJ youth attending physician, which notes the medications, treatment, and diet orders for the CDCR institution/DJJ facility and provides instructions to the CDCR/CCHCS patients and/or DJJ youth in order to maintain continuity of care. A transfer order is prepared when a CDCR/CCHCS patient and/or DJJ youth is discharged from the hospital and is returning to a CDCR institution/DJJ facility.
