When Advance Notice Is Not Possible. Advance written request for the use of PTO is not required if the employee’s own disability or an emergency necessitates an unplanned absence that was not requested and approved in advance of that day. In all such cases, the reason for the absence shall be given, and the employee shall follow the Medical Center’s or work area’s requirements as to when and how notice is to be given. If such unplanned absences (whether paid or unpaid) without advance notice and approval exceed five (5) incidents within the prior twelve (12) months, counseling or disciplinary action may be taken. Consecutive days taken at one time are one incident of unplanned absence.
