Written Application. Written application shall be made by the employee for such benefits to the Employer. In the case of absence for prolonged illness or industrial accident, the Employer's share of the premium will be paid to any of the above plans to a maximum of one (1) year from the commencement of the absence, or to the termination of the contract period in the case of a Temporary employee, if sooner.
