Written Questionnaire. 9 i. When a written statement of any kind is requested from an employee, 10 the employee shall be given the request in writing and the employee 11 shall to the best of his/her ability provide an accurate and truthful 12 written statement on the matter being investigated, including answers 13 to any specific questions included in the request. 14 ii. The employee shall be given a copy of the questionnaire and, if 15 available, sign for its receipt. 16 iii. The questionnaire shall contain questions pertaining to the incident 17 under investigation. [Note: When a critical or unusual incident report is 18 required, the employee may be required to provide a narrative 19 statement regarding the incident without the necessity of specific 20 written questions. Such report shall be provided promptly and 21 accurately to the best of the employee’s ability.] 22 iv. The employee shall be afforded a reasonable time to respond without 23 undue delay. When the employee is the subject of the investigation, 24 he/she shall have 24 hours to submit his/her response. 25 v. A copy of the written response shall be provided to the employee who 26 shall have 24 hours to review, amend, change or correct said 27 statement.
