This agreement confirms that Chestertown RiverArts is engaging said Instructor to teach the class(es) as outlined above on the dates listed. Instructor must be notified by the registration deadline if this class is to be canceled due to insufficient registration and agrees to submit a W-9 form in the event he/she receives $600 or more per calendar year in instructor payments.
