This End User Purchase, License and Services Agreement (the “Agreement”) is entered into as of the effective date specified above (the “Effective Date”) by and between FUJIFILM Medical Systems U.S.A., Inc., a New York corporation having a principal office at 81 Hartwell Avenue, Suite 300, Lexington, MA 02421 (“FMSU”), and the Customer specified above (“Customer”). The Agreement shall include all schedules and other attachments hereto, which are hereby incorporated herein and made a part hereof.
