This Agreement (“Agreement”) is made this Day of , 2018, by and between the State of Florida Department of Health, Pinellas County Health Department (“DOH-Pinellas”) and the City of St. Petersburg, Florida (“Provider”). The terms and conditions of this Agreement shall become a legal and binding agreement, which is sufficient to procure goods and/or services when this Agreement is signed by both parties (“Effective Date”) and shall expire on December 31, 2019.
