This Actuarial Services Contract (“Contract”) is made by and between Madison County Self-Insured Health Plan (the “Plan”), acting administratively through Madison County, Mississippi (“County”), and Cavanaugh Macdonald Consulting, LLC, (the “Actuary”), under the following terms and conditions under which the Actuary agrees to provide services to the Plan.
