THIS BUSINESS ASSOCIATE AGREEMENT (“BAA”) is entered into as of the last signature date below between MyHealth, LLC DBA ChartRequest (“Business Associate”) and (“Client” or “Covered Entity”) (each a “Party” and collectively the “Parties”). This BAA supersedes and replaces in their entirety all prior Business Associate Agreements between the Parties.
