THIS MASTER AGREEMENT (“Master Agreement” or “Agreement”), with an effective date of July 1, 2016, (hereinafter referred to as the “Effective Date”), is made and entered into by and between the Georgia Department of Public Health (hereinafter referred to as “the Department” or “Department”) and participating service providers.
